The SF12 module includes 12 questions - SCSF1 SCSF2A SCSF2B SCSF3A SCSF3B SCSF4A SCSF4B SCSF5 SCSF6A SCSF6B SCSF6C SCSF7
These questions are asked in the adult self-completion questionnaires.
Based on these questions two scores are derived: SF12MCS_DV & SF12PCS_DV
You can find these questions in the questionnaire, but in Waves 1 & 2, the adult self-completion questionnaires were provided in separate pdf files.
